  1. Madness classified by the vitiated dosha

    Unmāda is sorted into types according to which humour is deranged: vāta, pitta, kapha, all three together (sannipāta), or an exogenous cause such as possession by spirits. Each humoral type shows its own signature: vāta brings restlessness, pitta brings anger and heat, and kapha brings dullness and withdrawal. Treatment follows from the type, so diagnosis rests on reading the behaviour as evidence of a specific bodily imbalance rather than as a single undifferentiated illness.

  2. Mind disorder originates in bodily channels

    The text holds that vitiated doshas travel through the channels and afflict the heart, which it treats as the seat of the mind, and so obstruct the pathways of mental function. On this account insanity is a somatic derangement that happens to show itself in thought and conduct. Diet, oleation, purgation, emesis, and medicated ghee are therefore proper cures for a mental illness.

  3. Shock and fright as counter-therapy

    When purification and drugs fail, the text prescribes deliberate sudden fear, including threats, restraint, harsh words, and staged dangers, to jolt a deranged mind back into its proper state. The reasoning is that an intense counter-emotion can displace a disordered mental state, applying the principle of treating a condition with its opposite at the level of emotion. It also recommends consolation and pleasant stimuli for grief-based cases, so emotional intervention is matched to the patient's cause.
