1. The Night of the Turbine Rundown Test

   On 26 April 1986, Unit 4 at Chernobyl ran a test of whether a coasting turbine could power the cooling pumps. Deputy chief engineer Anatoly Dyatlov pressed the test ahead after the reactor had fallen to about 30 MW. Operators Leonid Toptunov and Alexander Akimov withdrew nearly all control rods to raise power, the emergency cooling system had been disabled, and when AZ-5 (the SCRAM button) was pressed at 1:23:40 the graphite-tipped rods caused a power surge that blew apart the reactor within seconds.

2. Design Flaws Turned Routine Actions Lethal

   Medvedev argues the disaster was not only operator recklessness. The RBMK had a positive void coefficient, so steam formation raised power instead of lowering it, and its control rods had graphite displacers that briefly added reactivity on insertion. Designers and ministry officials knew of these properties and did not tell the operators, so the emergency shutdown button itself set off the explosion.

3. Official Disbelief Delays the Response

   In the first hours, plant director Bryukhanov and the chain of command reported that the reactor was intact and radiation was modest. Instruments that read off the scale were treated as faulty, and people who reported graphite on the ground were not believed. Because each level told superiors what fit the accepted belief that 'an RBMK cannot explode,' the evacuation of Pripyat was delayed about 36 hours and firefighters were sent in without protection.

4. Normalized Violation Under Production Pressure

   A culture that rewards meeting plan targets makes breaking procedure routine: the test had already been postponed for grid demand, the crew improvised deviations from the written program, and safety systems were disabled so the schedule could hold. Individual rule-breaking reflected an industry where secrecy and output quotas outranked safety margins. Blame then fell on low-level operators rather than the institutions that set their incentives.
