The title sequence is a run of linked vignettes, mostly in the third person, set in a private Swiss clinic where patients are brought by spouses, lovers or friends and left in the institution's care. Several cling to a hope of rescue or a quick return that the reader can see is unlikely, though not all do: Hans fears having to leave. Committal often reads as a quiet desertion by those the patient trusted.
Several narrators face a remote board, committee or authority that never states the offence, the evidence or the rules. Because the accusation has no content, it cannot be properly answered or appealed. The narrator swings between protesting the injustice, trying to prove innocence and feeling a diffuse guilt, and reads delays and unexplained decisions as signs of the case going against her.
The people with power over the patient are experienced as adversaries: doctors, directors, lawyers and relatives who hold the key to release. Care and control are indistinguishable from the inside. Every protective act, such as sedation, supervision or withheld information, can be read as a move in a campaign against the self.
Many of the stories are told from inside a disturbed mind and give its persecutory reading of events a consistent logic, so the fear is not presented as plainly irrational. The book leaves it uncertain whether the persecution is real or delusional, and the third-person clinic scenes offer some outside view.
Narrators pin their hope of release on outside figures, such as an advocate, benefactor or influential patron, who might plead their case, so their fate depends on others' attention. These figures prove unreliable: some delay or drift away, and in 'Going Up in the World' a Patron and a Patroness humiliate and refuse the narrator outright. The failure becomes a further abandonment.
